What do I need to do to prepare for an initial appointment?
Filling out all intake forms to the best of your ability as well as releases and agreements will allow for a productive initial appointment. Please also have copies available of relevant evaluations or testing that you or your child have had in the past. Feel free to bring a friend or family member with you if you would find that helpful.
Where can I be seen for services?
Patty can work with clients in person in their homes or other locations. There is a travel charge for home visits if your location is 15 miles outside of Oxford, Pennsylvania.
Patty also works with clients via phone, Skype or other video conferencing platform.
Residents of Delaware may be eligible for telemedicine services.
Do I need a referral for services?
Patty is a licensed Occupational Therapist in the states of Pennsylvania and Delaware. If Occupational Therapy services are to be delivered in Pennsylvania, you will need a referral. In many states, such as Delaware, you do not need a referral, however, you would need one, if you plan to submit a bill to insurance or if you are using a Health Savings Account (HSA).
Referrals are not needed for if you will only be using Functional Medicine Health Coaching Services.
Who can write a prescription for Occupational Therapy?
In Pennsylvania, a licensed physician, physicians assistant, nurse practitioner, podiatrist, or optometrist can write a prescription for Occupational Therapy. The prescription needs to read, “OT evaluation and treatment” and needs to be accompanied with an ICD 10 diagnostic code.
What are your cancellation, no-show, and late appointment policies?
Please be courteous, and provide 48 hours notice for all cancelled appointments. This helps ensure that we can meet the needs of all of our clients who are waiting to get an appointment. There will be a $50 cancellation fee for no-shows. If you are late for your appointment by more than 15 minutes we will have to reschedule your appointment.
